What's L-PRF?

Leucocyte- and Platelet Rich Fibrin (L-PRF): the 2nd generation platelet concentrate. 

L-PRF is a modern blood derivate achieved with a simplified preparation, with no biochemical manipulation of blood.  This technique does not require anticoagulants, bovine thrombin or any other platelet activator agent.  This feature makes this product easily usable, with a low rate of mistakes during the preparation stage (Baeyens et al. 2010).  The blood, approximately 10 ml in sterile tubes without anticoagulant, is immediately centrifuged at 2700 rpm for 12 minutes.  The absence of anticoagulants allows the activation of platelets in contact with the inner walls of the tube, triggering the coagulation process.  The fibrinogen is initially concentrated in the upper part of the tube and the red blood cells in the lower part.  In the middle part, the platelets and leucocytes are retained into the meshes of fibrin.  A short time between blood collection and its centrifugation is most important to obtain a product clinically valid.  After centrifugation, the L-PRF cloth can be removed from the tubes with the use of surgical tweezers. The L-PRF cloth is carefully compressed (5 min) in order to obtain a strong and slightly elastic membrane, able to resist for example the pull of the suture.  L-PRF provides a scaffold consisting of fibrin that allows cellular migration, a fundamental aspect for the process of bone regeneration (Marrelli & Tatullo, 2013).  

L-PRF is a 3-D autogenous combination of Platelet Rich Fibrin derived from the patient’s blood. A simplified chairside procedure results in the production of a thin, compressed layer of platelet rich fibrin that is strong, pliable and suitable for suturing. This natural fibrin network is rich in platelets, growth factors and cytokines that are derived from the blood platelets and leukocytes. The presence of these proteins have been reported to produce rapid healing, especially during the critical first seven days after placement. This network promotes more efficient cell migration and proliferation without chemical or bovine thrombin additives.
